Separate a suggestion from an action
An assistant that drafts a response has a different responsibility from one that sends it or changes a business record. Define each permission explicitly.
Make review meaningful
A person should be able to see the proposed change, its context and its source information. A confirmation button without enough context is a weak review step.
Plan for uncertainty
Define how the system handles incomplete information, ambiguous instructions and failed requests. Give the user a clear route to correct or stop the work.
